Medicare Advantage HMO and PPO Options

HMOs generally use contracted networks and may require primary-care coordination or referrals. PPOs generally allow more out-of-network use at different costs, but noncontracted providers can decline routine care. Check every provider, pharmacy and prescription, plus prior authorization and formulary rules. The medical maximum out-of-pocket limit generally excludes Part D drug spending.

D-SNPs
