What are SSBCI Medicare benefits?

Special Supplemental Benefits for the Chronically Ill (SSBCI) are additional benefits offered by certain Medicare Advantage (Part C) plans. They may include services that are not primarily health-related when reasonably expected to improve or maintain an eligible member’s health or overall function. Read the federal SSBCI definition.

Possible benefitWhat a plan may cover
Healthy food and produceEligible groceries that support nutritional needs.
Utility assistanceQualifying electricity, gas, or water expenses.
Nonmedical transportationRides for activities such as grocery shopping.
Home supportCertain pest control services, indoor air-quality equipment, or structural modifications.
Examples of possible SSBCI benefits, not a guarantee of coverage.

Benefits, spending limits, and exclusions differ. Check the plan’s Evidence of Coverage before relying on a particular service. CMS’s SSBCI Guidance describes these benefit categories.

Who qualifies for SSBCI?

A member must meet all three parts of the federal definition:

  1. Have one or more comorbid and medically complex chronic conditions that are life-threatening or significantly limit overall health or function.
  2. Have a high risk of hospitalization or other adverse health outcomes.
  3. Require intensive care coordination.

A diagnosis alone is insufficient. The plan uses objective criteria to determine eligibility, which can differ by benefit. Social or financial needs alone do not establish eligibility. See federal eligibility requirements.

Is there a universal chronic conditions list?

No diagnosis checklist guarantees approval across all plans. Review each plan’s qualifying conditions and additional criteria. Ask how the plan evaluates your circumstances and what information it needs before choosing coverage. Read the rules on conditions and eligibility.

What changes for SSBCI in 2027?

CMS’s finalized 2027 rule requires plans to post their written SSBCI eligibility policies and objective criteria publicly. It also clarifies debit-card administration: cards must use a real-time mechanism to identify covered items and services at the point of sale, and benefits must be limited to the applicable plan year. CMS’s 2027 final-rule summary explains these changes.

Self-certifying that you qualify is insufficient, but this is not a new prohibition. CMS explains that plans may use health risk assessment responses as Part of an objective determination. The plan remains responsible for confirming eligibility before providing the benefit. See the final rule’s SSBCI administration discussion.

How do grocery or flex cards work?

Some plans administer benefits through a debit card. A card provides access to covered benefits under the plan’s rules; it does not establish eligibility on its own. CMS explains plan responsibility for card-based SSBCI.

Frequently asked questions

Does everyone on Medicare get SSBCI?

No. SSBCI is offered through certain Medicare Advantage plans and is limited to members who meet the plan’s eligibility requirements.

Does a chronic diagnosis automatically qualify me?

No. The plan must also determine that you face a high risk of hospitalization or other adverse health outcomes and require intensive care coordination.

Are SSBCI benefits the same as a Medicare flex card?

No. SSBCI describes a category of benefits. A flex or debit card is one way a plan may administer covered benefits; the card itself does not establish eligibility.

Can I qualify just by checking a box?

No. A plan must make an objective eligibility determination. Your health risk assessment responses may help the plan evaluate eligibility.

When do I enroll for coverage starting January 1, 2027?

The Annual Enrollment Period runs October 15 through December 7, 2026. Other enrollment opportunities depend on your circumstances.

Can an insurance agent approve my SSBCI benefits?

No. An agent can explain plan rules and help with enrollment, but the insurance plan determines eligibility.
