Practically: choose Medicare Advantage at 65, want Medigap at 70, and the insurer can ask health questions and decline you. Narrow federal guaranteed-issue rights exist, including a trial right in your first year on Medicare Advantage, but they are exceptions rather than a safety net.

If you are in the Qualified Medicare Beneficiary program, federal law prohibits providers from billing you for Medicare deductibles, coinsurance or copayments. Not reduces — prohibits, whether or not the provider participates in Medicaid.

It is also one of the most frequently violated rules in the system. Bills arrive, they look official, and people pay them. If it happens, tell the provider you are a QMB and that balance billing is prohibited under federal law, and ask for a refund of anything already paid.

Keep the letter confirming your status somewhere you can find it. Producing the document usually ends the conversation, and in a county where one beneficiary in four is dual eligible this comes up more often than it should.
