It applies only to the same company or an affiliate authorised in Illinois, and it is restricted to people aged 65 through 75 — at 76 the window closes for good. The rule was revised with effect from 1 January 2026 under Illinois Public Act 103-0747.

It does not let you switch to a different insurance company, and it does not let you upgrade to richer benefits. Both of those still require medical underwriting outside a guaranteed-issue situation. If someone tells you Illinois lets you freely shop Medigap every year, they are describing a rule that does not exist.

Your one unrestricted opportunity is still the six-month Medigap Open Enrollment Period that begins the first month you are both 65 or older and enrolled in Part B. It does not repeat.

Illinois is also more generous than most states to people who qualified for Medicare under 65 through disability, a group that has no realistic Medigap option in much of the country.

Prescription costs and the 2026 cap

For most people the drug list, not the premium, decides which of the Medicare plans Oak Park offers is right.
