This page explains how Anthem Blue Cross Medicare plans work, what to check before you enroll, and how Anthem Blue Cross Medicare plans compare with everything else available in your county. We are an independent agency, not the carrier.
Understanding Anthem Blue Cross Medicare plans
“Anthem” is a familiar national brand, but insurance is issued by state-specific companies. Anthem's public legal language identifies different underwriting and HMO entities in Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, New York, Ohio, Virginia and Wisconsin. California generally uses the Anthem Blue Cross name.
The Blue license territory matters. Anthem's Missouri business excludes 30 counties in the Kansas City area. Its New York Anthem Blue Cross and Blue Shield territory covers 17 southeastern counties. The Virginia service area excludes specified Northern Virginia locations. Other Blue plans may serve excluded territories.
That is why a state name alone is not enough. A licensed O'Neal Insurance Group agent can help you compare insurance companies and plans currently available in your ZIP code and county. The official plan document must identify the legal insurer, service area and provider network.

Anthem HMO and PPO plans

An Anthem Medicare Advantage HMO generally relies on a contracted provider network. Members may select a primary care provider, and referrals or prior authorization may be required for certain services. Care outside the network usually has limited coverage except for emergencies, urgent care and other circumstances defined by the plan.
An Anthem PPO generally offers more out-of-network flexibility. That flexibility can come with higher cost sharing, and a non-network provider may not agree to furnish non-emergency care. Verify each provider under the exact plan name—not simply whether the office “takes Anthem.”
Medicare Advantage plans have an annual maximum out-of-pocket limit for covered Parts A and B services. Prescription expenses normally follow separate Part D cost rules. Review the Summary of Benefits, Evidence of Coverage, formulary and provider directory before enrolling.
Special Needs Plan guidance
D-SNPs
An Anthem D-SNP may serve people with Medicare and a qualifying level of Medicaid in a documented service area. Medicaid categories can affect member cost sharing and available coordination. An insurance agent may explain the requirements, but the state Medicaid agency makes the official Medicaid determination.
C-SNPs
A C-SNP focuses on one or more qualifying chronic conditions. Medicare-market examples can include diabetes, cardiovascular disorders or chronic heart failure. The existence of a condition does not prove an Anthem C-SNP is offered in the county. Enrollment generally requires the qualifying condition to be confirmed.
I-SNPs
Discuss an Anthem I-SNP only if current CMS and carrier documents identify one in the service area. Institutional residence or an institutional-equivalent level of need and other plan requirements may apply.
Medicaid, Medicare Savings Programs and Extra Help

Anthem participates in certain state Medicaid programs, but Medicaid product states are not the same as Anthem's entire commercial or Medicare footprint. Eligibility is decided by the applicable state agency.
Medicare Savings Programs may help eligible beneficiaries with certain Medicare premiums and cost sharing. Extra Help—also called the Low-Income Subsidy or LIS—may help eligible people with Part D premiums, deductibles and prescription cost sharing. These are government programs, not automatic Anthem benefits.
An agent can help you understand how a documented Medicaid or Extra Help status may affect plan comparison, but cannot approve the assistance.
Part B givebacks, flex cards and additional benefits
Certain Anthem Medicare Advantage plans may include a Part B premium reduction. It reduces the amount paid toward the Part B premium under the plan's terms; it is not cash. A flex or allowance card, when included, has approved categories, participating locations and usage periods.
Dental, vision, hearing, fitness, transportation, food, utility or over-the-counter benefits can differ across plans and may require particular eligibility.
Part B premium reductions, flex cards, and other supplemental benefits are not included with every Medicare Advantage plan. Availability, eligibility, benefit amounts, approved expenses, and participating locations vary by plan and service area.
Anthem coverage and veterans
VA health care, TRICARE and Medicare are separate systems. A veteran may be able to keep VA health care while enrolling in a Medicare plan, but should consider non-VA providers, prescriptions, referrals, urgent care and emergencies. Medicare Advantage does not replace VA benefits.
People with TRICARE for Life should confirm how any plan or Part D coverage coordinates with TRICARE. The VA or TRICARE should confirm decisions that could affect military benefits.
Non-Medicare Anthem coverage
Anthem offers individual/family and employer health insurance in applicable Blue territories. Depending on the state, Anthem affiliates may also offer Medicaid, dental and vision products. These products are separate from Medicare.
An individual dental policy is not Part of Original Medicare. Medicaid eligibility is determined by the state. Employer coverage is governed by the employer's plan documents. Compare each category under its own rules.

Who may qualify for an Anthem D-SNP?
A person generally needs Medicare, a qualifying Medicaid status, residence in the service area and satisfaction of the plan's requirements.
What may qualify someone for a C-SNP?
The person must have a chronic condition covered by a C-SNP offered in the county, and the condition generally must be verified.
Does Anthem offer Medicare Supplement coverage?
Anthem affiliates document Medicare Supplement products in multiple states. Available letter plans, premiums and underwriting rules vary.
Does every Anthem plan include a flex card or Part B giveback?
No. These benefits are plan-specific and must be verified in current official documents.
Can veterans retain VA health care?
Medicare and VA care are separate. Veterans should review how prescriptions, non-VA providers and emergency care will work before enrolling.

How to compare Anthem Blue Cross Medicare plans against the alternatives
No carrier is the right answer for everybody, and that includes Anthem Blue Cross Medicare plans. What decides it is your own doctors, your own prescriptions and how you prefer to pay, in that order.
Start with your providers. Check every physician you want to keep against the plan directory, and confirm the medical group as well as the individual name. That single step rules Anthem Blue Cross Medicare plans in or out faster than any brochure will.
Then price your prescriptions. Two plans with an identical premium can differ by thousands of dollars a year once your real drug list is run against each formulary. Ask specifically about tiers, step therapy and prior authorization.
Then look at the ceiling, not the premium. Every Medicare Advantage plan carries an annual out-of-pocket maximum; Original Medicare on its own does not. That ceiling is the real protection, and it varies by thousands of dollars between plans.
Commissions are set by the Centers for Medicare & Medicaid Services and are identical across carriers for the same product line. An independent agent comparing Anthem Blue Cross Medicare plans with everything else available in your county therefore has no financial reason to favor one over another.
What 2026 changes for Anthem Blue Cross Medicare plans
Two federal rules apply to every Part D policy this year, Anthem Blue Cross Medicare plans included, whether standalone or bundled inside a Medicare Advantage plan. No plan may charge a drug deductible above $615, and once you have spent $2,100 out of pocket on covered drugs your cost for the rest of the calendar year drops to zero.
The Medicare Prescription Payment Plan lets you spread that $2,100 across monthly installments instead of facing a large bill in January. It is worth asking about if your medications are expensive early in the year.
What still changes annually is the detail: networks, formularies, copays and supplemental benefits. The Annual Notice of Change your carrier sends by September 30 is where those changes are disclosed, and it is the document to read before deciding whether to keep Anthem Blue Cross Medicare plans for another year.
Enrollment windows that apply to Anthem Blue Cross Medicare plans

The windows below govern Anthem Blue Cross Medicare plans exactly as they govern every other Medicare Advantage and Part D plan. They are federal, not carrier-specific, and no salesperson can extend one for you.
Initial Enrollment Period. Seven months centred on the month you turn 65 — the three months before, the birthday month itself, and the three months after. Part B and Part D late penalties are permanent, so this is the window that costs money to miss.
Annual Enrollment, October 15 to December 7. Open to everyone on Medicare. Whatever you choose takes effect on January 1.
Medicare Advantage Open Enrollment, January 1 to March 31. One change only, and only if you are already enrolled in an Advantage plan.
Special Enrollment Periods. Moving out of a plan’s service area, losing employer coverage, entering or leaving a nursing home, or gaining or losing Medicaid each open a window outside the normal calendar.
