This page explains how ManhattanLife insurance plans work, what to check before you enroll, and how ManhattanLife insurance plans compare with everything else available in your county. We are an independent agency, not the carrier.

ManhattanLife is a brand, not one universal policy issuer

ManhattanLife is the official brand spelling used by a group of life and health insurance companies. Depending on the product and state, an application may name Manhattan Life Insurance Company, ManhattanLife Insurance and Annuity Company, ManhattanLife Assurance Company of America, Standard Life and Casualty Insurance Company or another disclosed affiliate.

That legal-company detail matters. The underwriting company issues the contract, collects premiums and is responsible for covered claims. A product approved under one ManhattanLife entity in Alabama may use a different entity or may not be open for new applications in California, New York or another state.

ManhattanLife publishes a Product Approval Chart. The version reviewed was dated July 2026 and covers the states, District of Columbia and Puerto Rico. Because the chart contains product-by-product marks and notes that the underwriting company varies by state, consumers and agents should check the current chart and state-specific application immediately before quoting. A map is evidence of approval at the time published, not a promise that every applicant qualifies.

ManhattanLife products and state availability

Official materials identify the following categories. Each is available only in the states and through the underwriting companies shown on the current Product Approval Chart:

  • Medicare Supplement: standardized Medigap policies used with Original Medicare. Current states and plan letters must be checked in the live product map.
  • Dental, Vision & Hearing and DVH Select: stand-alone supplemental coverage, with options and limits that vary by form and state.
  • Hospital Indemnity Select and Affordable Choice: fixed-indemnity products that pay according to covered events and schedules rather than replacing major medical insurance.
  • Cancer, Heart and Stroke and Critical Protection & Recovery: specified-disease or critical-illness coverage with benefit triggers defined by the contract.
  • Home Health Care and Home Health Care Select: limited-benefit coverage for qualifying home-care services under policy rules.
  • Short-Term Care: coverage for qualifying facility or home care for a limited benefit period.
  • Accident products: benefits for covered accidental injuries and services.
  • Final expense and other life insurance: death-benefit coverage issued under the applicable life policy.
  • Disability and employer-group products: income-protection and voluntary workplace benefits where approved.
  • Annuities: financial contracts that are not health insurance or Medicare.

Comparing ManhattanLife insurance plans

Medicare Supplement insurance, also called Medigap, works with Original Medicare Parts A and B. Medicare pays first for covered services, and the Medigap policy helps with certain deductibles, copayments or coinsurance according to the selected standardized plan.

Medigap is not Medicare Advantage. It does not use an HMO or PPO network. In general, a beneficiary may use any provider that participates in Medicare, subject to Original Medicare rules. Modern Medigap policies do not include outpatient prescription drug coverage, so a person who needs drug coverage usually compares a separate Part D plan.

Standardized plan letters provide the same core benefits regardless of insurer in most states, but premiums, rating method, underwriting, discounts and customer service can differ. Massachusetts, Minnesota and Wisconsin standardize Medigap differently. Plans C and F are generally unavailable to people newly eligible for Medicare on or after January 1, 2020.

The best time to purchase Medigap is often the six-month Medigap Open Enrollment Period that begins when a person is 65 or older and enrolled in Part B. Outside protected periods, medical underwriting may apply unless federal or state guaranteed-issue rights protect the applicant. Do not cancel existing coverage until the new policy has been issued and its effective date is confirmed.

ManhattanLife is not verified as a Medicare Advantage or Part D carrier

Current sources reviewed do not establish ManhattanLife as a 2026 Medicare Advantage, HMO, PPO, D-SNP, C-SNP, I-SNP or stand-alone Part D organization. Therefore, Part B givebacks, flex cards, Medicare Advantage dental benefits and provider networks should not be described as ManhattanLife benefits.

A beneficiary can compare ManhattanLife Medigap while separately reviewing Part D plans from approved prescription drug carriers. Someone considering Medicare Advantage would instead compare plans contracted for the person's county. Medigap cannot be used to pay Medicare Advantage cost sharing.

Dental, vision and hearing coverage

Original Medicare generally does not cover routine dental services, eye exams for glasses or hearing aids. A ManhattanLife Dental, Vision & Hearing policy may help with covered services where the product is approved, but it is separate insurance.

Check the policy's deductible, waiting periods, annual benefit maximum, covered-service schedule, network arrangements, reimbursement method and exclusions. “Choose any dentist” language does not mean every dentist bills the carrier directly or that the policy pays the full charge. For vision and hearing, verify examination, eyewear, contact-lens and hearing-aid limits separately.

Hospital indemnity and gap-style products

Hospital Indemnity Select, Affordable Choice and related fixed-indemnity products are designed to pay stated benefits after covered events. They do not promise to pay the entire hospital bill and are not substitutes for major medical insurance, Medicare or Medicare Advantage.

Compare the hospital-admission definition, daily confinement schedule, observation-status rules, outpatient benefits, waiting periods, exclusions and optional riders. Ask whether benefits are paid to the insured or assigned elsewhere and what documents are required for a claim. A policy can be useful only if its benefit structure matches the risk the household wants to address.

Cancer, heart attack, stroke and critical illness coverage

Specified-disease and critical-illness policies pay only when the diagnosis or procedure meets the contract definition. A general history of heart disease, for example, does not guarantee payment for every cardiac event. Waiting periods, survival periods, recurrence provisions, restoration riders and pre-existing-condition rules can apply.

ManhattanLife's 2026 product announcements show that some riders roll out as states approve them. That is why a national product name cannot replace a current state check. Read the state-specific policy and outline of coverage before applying.

Home health care and short-term care

Home health care insurance may pay benefits when covered services and eligibility triggers are met. Short-term care may cover qualifying facility or home care for a limited period. Neither is comprehensive long-term care insurance, Medicare home health coverage or Medicaid.

Review activities-of-daily-living triggers, cognitive-impairment terms, elimination periods, benefit days, care-provider qualifications, prescription or caregiver features, inflation options and exclusions. Family caregiving arrangements should be confirmed in writing because contracts may define eligible providers differently.

Life and final expense insurance

Final expense insurance is generally a form of permanent life insurance intended to provide a death benefit. Beneficiaries may use policy proceeds according to the contract; the product is not Medicare and does not pay medical claims like health insurance.

Compare the death benefit, premium schedule, issue ages, underwriting, graded or limited-benefit periods, cash value, riders and beneficiary designations. A “no medical exam” application can still include health questions or policy limitations. Review replacement consequences before exchanging an existing life policy.

Veterans, Medicaid and financial-assistance programs

VA health care, TRICARE, Medicare and Medicaid are separate systems. ManhattanLife supplemental policies do not replace military benefits or establish Medicaid eligibility. Veterans should confirm how any Medicare choice affects prescriptions, non-VA providers and TRICARE before enrolling.

Extra Help and LIS are two names for the same federal Part D subsidy. Medicare Savings Programs are state-administered. Neither program is a ManhattanLife policy benefit, and an insurance agent does not decide eligibility. A Medigap applicant receiving Medicaid should first review whether Medigap is appropriate and permitted for the person's situation.

How to compare ManhattanLife insurance plans against the alternatives

No carrier is the right answer for everybody, and that includes ManhattanLife insurance plans. What decides it is your own doctors, your own prescriptions and how you prefer to pay, in that order.

Start with your providers. Check every physician you want to keep against the plan directory, and confirm the medical group as well as the individual name. That single step rules ManhattanLife insurance plans in or out faster than any brochure will.

Then price your prescriptions. Two plans with an identical premium can differ by thousands of dollars a year once your real drug list is run against each formulary. Ask specifically about tiers, step therapy and prior authorization.

Then look at the ceiling, not the premium. Every Medicare Advantage plan carries an annual out-of-pocket maximum; Original Medicare on its own does not. That ceiling is the real protection, and it varies by thousands of dollars between plans.

Commissions are set by the Centers for Medicare & Medicaid Services and are identical across carriers for the same product line. An independent agent comparing ManhattanLife insurance plans with everything else available in your county therefore has no financial reason to favor one over another.

What 2026 changes for ManhattanLife insurance plans

Two federal rules apply to every Part D policy this year, ManhattanLife insurance plans included, whether standalone or bundled inside a Medicare Advantage plan. No plan may charge a drug deductible above $615, and once you have spent $2,100 out of pocket on covered drugs your cost for the rest of the calendar year drops to zero.

The Medicare Prescription Payment Plan lets you spread that $2,100 across monthly installments instead of facing a large bill in January. It is worth asking about if your medications are expensive early in the year.

What still changes annually is the detail: networks, formularies, copays and supplemental benefits. The Annual Notice of Change your carrier sends by September 30 is where those changes are disclosed, and it is the document to read before deciding whether to keep ManhattanLife insurance plans for another year.

Enrollment windows that apply to ManhattanLife insurance plans

The four Medicare enrollment windows

The windows below govern ManhattanLife insurance plans exactly as they govern every other Medicare Advantage and Part D plan. They are federal, not carrier-specific, and no salesperson can extend one for you.

Initial Enrollment Period. Seven months centred on the month you turn 65 — the three months before, the birthday month itself, and the three months after. Part B and Part D late penalties are permanent, so this is the window that costs money to miss.

Annual Enrollment, October 15 to December 7. Open to everyone on Medicare. Whatever you choose takes effect on January 1.

Medicare Advantage Open Enrollment, January 1 to March 31. One change only, and only if you are already enrolled in an Advantage plan.

Special Enrollment Periods. Moving out of a plan’s service area, losing employer coverage, entering or leaving a nursing home, or gaining or losing Medicaid each open a window outside the normal calendar.
